2. Send application and checks payable to Foresight Design Initiative to:

3. Upon receipt of your completed form and payment, you will be subscribed to the moderated, member-only email list, and a new member package will be sent to you.
4. Contact information and sustainability activities from your application will be used to compile your profile to the online directory. Once posted, you will receive a notice to review it for accuracy and/or edits.

Annual/Monthly Payment
Members may pay annual dues in a one-time installment, or may opt to make 12 monthly installments charged to a credit card. In order to account for extra time and fees for processing monthly payments, divide the above levels by ten. (For example: Implementer level: $50/month for 12 months.)
In-Kind Donation Recognition
Initial membership dues must be paid in dollars annually. An in-kind donation may be acceptable to increase your membership level in any year. In-kind donations will be negotiated on a case-by-case basis.
